Home Health RN Responsibilities
- Perform comprehensive patient assessments and ongoing skilled nursing visits.
- Develop, implement, and update individualized plans of care.
- Administer medications, wound care, IV therapy, and other skilled nursing treatments as ordered.
- Monitor patient progress, identify changes in condition, and communicate updates with physicians and the interdisciplinary care team.
- Educate patients and caregivers on disease management, medications, treatments, and safety in the home.
- Complete accurate, timely clinical documentation in accordance with agency, state, and federal guidelines.
- Coordinate care and facilitate appropriate referrals to ensure optimal patient outcomes.
Requirements
- Active Pennsylvania RN License
- Minimum of 2 years of Home Health or Acute Care RN experience
- Reliable transportation
- BSN preferred
