• Full-time schedule, Monday through Friday 8a-5p

• Provide hands-on, personalized care in patients’ homes

• Manage and coordinate a caseload from start to finish

• Perform assessments and develop individualized care plans

• Monitor patient progress and adjust care as needed

• Collaborate with physicians and interdisciplinary teams

• Educate patients and families for long-term success

• Ensure compliance with all clinical and regulatory standards (including HIPAA)

• Active, unrestricted RN license

• 1+ year of clinical nursing experience

• Ability to travel within the coverage area (up to 50%)

• Reliable transportation, valid driver’s license & insurance

• Strong clinical judgment and communication skills

• Ability to lift, move, and assist patients (50–100 lbs intermittently)

Preferred:

• Home Health or Hospice experience

• Experience with HomeCare HomeBase

• ADN or BSN
