Registered Nurse (RN) – Home Health Case Manager

What You’ll Do

As a Home Health RN, you’ll be the heart of patient care—bringing clinical excellence directly into patients’ homes.

  • Manage a dynamic caseload of patients in your local community

  • Perform comprehensive assessments and develop individualized care plans

  • Provide hands-on skilled nursing care and patient education

  • Monitor patient progress and adjust care plans as needed

  • Collaborate with physicians, therapists, and interdisciplinary teams

  • Be a trusted partner for patients and families throughout their care journey

What You’ll Need
  • Active, unrestricted RN license

  • At least 1 year of clinical nursing experience

  • Ability to travel locally (you’ll spend time in the field—this is not a desk job)

  • Valid driver’s license, reliable vehicle, and current auto insurance

  • Ability to safely assist patients (lifting, positioning, etc.)

  • Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.

Preferred (but not required):

  • Home Health or Hospice experience

  • Familiarity with HomeCare HomeBase

  • Associate’s or Bachelor’s Degree in Nursing
