Full Time/ Springfield / $88k-$97k Guaranteed Salary + Productivity Bonus

Looking for a nursing role where you can use your clinical judgment, build relationships, and manage your day? Join Elara Caring as a Home Health RN Case Manager serving patients throughout the Springfield area.

• Guaranteed salary of $88,000- $97,000, based on experience, plus a productivity bonus

You’ll guide patients through their home health care, from assessment to discharge. That includes providing skilled nursing visits, developing and updating care plans, managing an assigned caseload, and coordinating with physicians and other clinicians. You’ll also teach patients and caregivers how to manage medications, treatments, and health conditions at home.

• Current, unrestricted Massachusetts RN license
• At least one year of clinical nursing experience
• Strong assessment, communication, and time management skills
• Ability to work independently and travel throughout Springfield
• Valid driver’s license, current auto insurance, and reliable transportation
• Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 pounds
