Description
- Assess, evaluate, and develop discharge plans for patients.
- Coordinate services to ensure patients are moved to appropriate levels of care.
- Initiate, evaluate, and re-evaluate treatment plans daily.
- Monitor patient progress and address lack of progress on a daily basis.
- Track length of stay and identify barriers to timely discharge.
- Follow Medicare regulatory guidelines, including patient choice, IMM notices, and MOONS.
- Work schedule is Monday–Friday; no weekends or on-call required.
- May be required to float within the facility or within scope of practice as needed.
- Minimum of 3 years of experience required; not open to first-time travelers.
- Clinical screening required to determine fit; final decision made by manager.
- Maximum of 7 days of requested time off (RTO) allowed; no RTO permitted in September.
- Expected to work major holidays as per assignment period guidelines.
- Full compliance with orientation and onboarding requirements necessary for timely start.
- Local applicants accepted at same rate.
- Non-billable orientation hours determined by contract length.
