Location: Santa Rosa, California
Compensation: Details provided to qualified applicants
Schedule: Full-time travel assignment; patient ratio 1:20; 2 weekends every 6 weeks; no call required; floating between units based on departmental needs

Requirements:

• Active RN license
• BLS certification required
• Minimum 2 years of case management experience
• Current work history within the last 2 years (required)
• Epic charting system experience required
• Travel nursing experience preferred
• Community hospital, LTAC, or Trauma Level I/II experience preferred
• Skills in discharge planning, care coordination, medical necessity determination, and utilization of InterQual criteria
• Competency in arranging DME, SNF placements, and ambulance transport independently
• Ability to organize and participate in family meetings and treatment plan development
• Experience working with HMOs, IPAs, and insurance processes
