Job Details

• Weekly Estimated Pay: $2,490 - $2,960
• Location: Burlingame, CA 94010 (San Francisco Peninsula)
• Employment Type: Travel Contract (Travel RN / Travel Nursing)
• Duration: 13 weeks
• Hours per Week: 40
• Shift: 8-hour day shifts, variable start times between 7:00am and 3:30pm
• Shifts per Week: 5
• Schedule: Every other weekend required; floating to various units as needed
• Setting: On-site at a leading medical center (acute care hospital)

Required Qualifications

• Active RN License (California)
• BLS Certification (AHA)
• Minimum 5 years of acute care nursing experience
• CCM (Certified Case Manager) or ACM certification preferred
• Experience with Epic EMR (electronic medical record)
• Ability to be cross-trained on all units and float as needed

Key Responsibilities

• Provide comprehensive case management services across multiple acute care units as a Case Manager RN
• Coordinate patient care, utilization review, and discharge planning to support timely transitions of care
• Collaborate with interdisciplinary teams (physicians, social work, rehab, care coordination) to ensure optimal patient outcomes
• Document care plans and interventions accurately using Epic EMR
• Adapt to variable start times and float to different units as required by the travel assignment
• Work every other weekend as part of the rotation and support continuity of care
