Travel RN Case Manager Job Details

Job Requirements — Case Management RN / Travel RN

• Active California RN license (required)
• BLS certification (AHA)
• Minimum 1 year of acute care RN experience (2+ years preferred)
• Strong critical thinking, communication, and care coordination skills
• Proven ability to multitask in a fast-paced hospital environment
• Experience with interdisciplinary teamwork and clinical case management
• Preferred: BSN or Master’s degree
• Preferred: Case management certification (CCM, ACM)
• Preferred: Experience with managed care, utilization review, and discharge planning

Responsibilities — RN Case Manager

• Manage patient care across the continuum from admission through discharge, ensuring safe transitions and continuity of care
• Develop and implement individualized care plans focused on outcomes and resource utilization
• Coordinate safe and timely discharges to home, skilled nursing facilities, or rehab facilities
• Ensure appropriate level of care and participate in utilization review processes
• Collaborate with physicians, nursing staff, social workers, utilization review teams, and ancillary departments
• Communicate effectively with insurance providers, case managers, and post-acute services
• Advocate for patient and family needs, addressing barriers such as language, cultural, or socioeconomic challenges
• Apply clinical guidelines to ensure appropriate care levels and document clinical justifications in Epic
• Track patient progress and outcomes, adjusting care plans and coordinating follow-up care as needed
• Support care coordination initiatives, discharge planning, and managed care activities to optimize utilization and patient outcomes
