Job Details
- Position: Registered Nurse (RN) – Case Management / Pediatric Case Manager
- Pay: $2,754 - $3,060 weekly (estimated)
- Location: Palo Alto, CA (Zip: 94304-1601) — Silicon Valley, California nursing jobs
- Setting: On-site at a leading children's hospital (pediatric nursing job)
- Contract Duration: 13 weeks to start, with likely extension up to 6 months
- Schedule: 4 shifts per week, 10-hour days

Qualifications / Job Requirements
- Active Registered Nurse (RN) license in California
- Minimum 1 year of recent case management experience (RN Case Manager)
- Experience in Utilization Review (UR) and Discharge Planning (DP) preferred
- Strong communication, patient advocacy, and organizational skills
- Ability to work effectively in a multidisciplinary team (physicians, social workers, therapists)
- Pediatric case management or pediatric nursing experience is a plus for children's hospital roles
Responsibilities
- Coordinate and manage individualized care plans and care coordination for pediatric patients
- Conduct utilization review (UR) and discharge planning (DP) to ensure safe, timely transitions and optimal patient outcomes
- Serve as a patient and family advocate, connecting families with hospital and community resources
- Collaborate closely with physicians, social workers, and interdisciplinary team members to support care delivery
- Maintain accurate documentation, complete required forms, and comply with hospital policies and regulatory standards
