Job Description

• Coordinate management of care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
• Provide ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and evaluation of individual patient needs.
• Enhance quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness by integrating case management, utilization review, and discharge planning functions.
• Serve as a patient advocate by addressing holistic patient needs, resource utilization, and discharge planning.

Requirements

• Specialty: Registered Nurse (RN) - Case Management
• Years of Experience: Minimum 2 years recent RN Case Management and discharge planning experience in a hospital setting (higher value between 1 and 2 years applied)
• Licensure: Must have a valid California RN license
• Certifications: BLS certification through the American Heart Association required
• Shift: Monday - Friday, 8:00 AM to 4:30 PM
• Prior Travel Experience: Not explicitly required
