Job Opportunity: RN - Case Management (Travel)

Job Description

• Coordinate management of care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
• Provide comprehensive assessment, care planning, plan implementation, and evaluation of individual patient needs.
• Enhance quality of patient management and satisfaction, promoting continuity of care and cost effectiveness.
• Integrate case management, utilization review, and discharge planning functions.
• Serve as a patient advocate addressing holistic patient needs through resource utilization and discharge planning.
• Collaborate strongly with interdisciplinary teams (IDT).

Schedule

• Monday to Friday, 8:00 AM – 4:30 PM

Contract Length

• 13 weeks

Required Qualifications

• License: California RN license required.
• Certifications: BLS certification through the American Heart Association required.
• Experience: Minimum 2 years recent RN Case Management experience required.
• Experience must include LTAC or STACH settings, MCG guidelines, and discharge planning across acute care settings.
• Strong interdisciplinary team collaboration skills required.
