Description

Registered Nurse (RN) - Acute Inpatient Rehab

Location: Waco, Texas
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 6:45 PM – 7:15 AM


Job Description

  • Provide direct patient care in an inpatient rehabilitation setting.
  • Implement individualized care plans.
  • Coordinate patient discharge needs.
  • Preferred experience in inpatient rehab nursing.

Required Qualifications

  • Valid RN license in Texas
  • Associate of Science in Nursing / Associate Degree in Nursing
  • Minimum of 2 years of rehabilitation nursing experience, including experience within the last year
  • Basic Life Support (BLS) certification issued by the American Heart Association
  • NIH Stroke Scale (NIHSS) certification

Onboarding & Compliance Requirements

Professional Documentation (Required for Submission)

  • Resume or professional profile.
  • Two professional references.
  • Driver’s license or state ID (valid, not expired, matching address requirements).
  • License/certificate primary source verification.
  • Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others within 90 days of assignment start.
  • Employment verification for the past seven years.
  • Education verification relevant to clinical work within 90 days of start.

Required Onboarding Documents & Testing (Must be completed prior to or at start)

  • Signed assignment agreement (offer letter, initial confirmation, and extensions).
  • Ascension Specialty Exam (minimum 80% passing score; updated every 3 years).
  • Ascension Core Competencies (HIPAA, Joint Commission, OSHA, etc.; minimum 80% passing score; updated every 3 years).
  • Ascension Nursing Contract Worker Onboarding (completed onsite).
  • Ascension Epic Test-Out (Epic training and test results with signed attestation).
  • Ascension Color Vision test (updated every 3 years).
  • NIH Certification (Group A updated yearly; Groups B-F every 2 years).
  • Ascension Onboarding Education Attestation (completion of pre-hire education modules).
  • Ascension Nova StatStrip pre-hire modules.
  • Ascension Skills Checklist (clinical skills; updated annually).
  • Ascension Criminal Background Check and EPLS (updated annually or if break in service > 90 days).
  • Ascension Confirmation of Assignment (initial and updated with extensions).
  • Ascension Employment Verification (updated annually or if break in service > 90 days).

Health & Immunization Requirements (Must be current and documented)

  • MMR: Proof of two vaccines or positive IGG titer; no declinations accepted.
  • Varicella: Proof of two vaccines, serological immunity, or documented history; no declinations accepted.
  • Tdap: Valid within 10 years; no declinations accepted.
  • Hepatitis B: Vaccination series completion or signed declination.
  • Influenza vaccine: Optional; if declined during flu season, mask required.
  • Tuberculosis screening: 2-step PPD or IGRA within required timeframes; if positive, chest X-ray and annual TB questionnaire.
  • Fit Test: Required for all travelers; preferred before start, can be completed onsite.
  • Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
  • 12 Panel Drug Screen: Within 30 days prior to start, updated annually; includes testing for 12 specified substances.

Additional Notes

  • Optional: Badge photo (forward-facing headshot with white or blue background, no filters).
  • Optional: Unit-specific checklist may be required by unit leader within one week of start.
  • Optional: Hiring manager evaluation and name change documentation.

This position requires a dedicated RN with inpatient rehab experience, strong compliance with onboarding and health requirements, and the ability to work overnight shifts in a long-term assignment setting.
