Registered Nurse (Home Health / Hospice) - Wisconsin

Job Details

  • Location: Beaver Dam, Wisconsin
  • Contract Length: 13 weeks
  • Shift: Days (Monday through Friday, 8:00 AM – 4:30 PM, end time may vary)
  • Hours per Week: 40 hours
  • Call Requirement: On call 2 times per week, Monday through Thursday
  • Weekend Requirement: Weekends only during on-call shifts
  • Floating: No floating; will cover both home health and hospice visits
  • Patient Load: 5-6 patients per day, approximately 25-30 points per week (routine and OASIS visits)

Patient Population & Care Focus

  • Primarily geriatric patients with occasional pediatrics
  • Common diagnoses include:
    • Heart Disease
    • Chronic Respiratory Diseases (CHF, COPD, Pneumonia)
    • Dementia/Alzheimer’s
    • Depression
    • Stroke
    • Cancer
    • Kidney Disease
    • Diabetes
    • Liver Disease
    • Wounds, Joint Replacement, Surgery
  • Care includes medication management, catheter management, complex wound care, pain and symptom management, and end-of-life care

Required Skills & Experience

  • Prior travel nursing experience required
  • Critical thinking abilities
  • Chronic disease management
  • Patient and family education
  • Experience with:
    • Negative Pressure Wound Therapy (NPWT)
    • Ostomy care
    • Central line care
    • IV administration
    • Complex wound care
  • Hospice experience including pain and symptom management, empathy, and compassion

Required Certifications & Licenses

  • Current Registered Nurse (RN) license for Wisconsin
  • Basic Life Support (BLS) certification (AHA)

Orientation & Training

  • 1 day computer-based training
  • 2-5 days field training/shadowing depending on experience

Work Environment & Dress Code

  • Solid color scrubs only (no patterns or logos unless company-issued)
  • Closed-toe, non-slip shoes with socks or hosiery required
  • No flip-flops, open-toe shoes, or sandals allowed in patient care areas

Additional Information

  • Weekly interdisciplinary team (IDT) meetings and case conferences at the office
  • Monthly all-staff meetings
  • No ventilator patients; occasional CPAP and BiPAP patients who self-manage
  • Certified nursing assistants available for support
  • Clinicians must clock in and out at the facility
  • If previously employed by this organization, must have ended employment at least 6 months prior to submission date
