Job Opportunity: Rehab Nurse (RN Acute Rehab)
Location: Evansville, Indiana
Employment Type: Long-Term Assignment (13 weeks)
Shift: Night Shift, 6:45 PM – 7:15 AM
Unit: Acute Rehab / Post Acute
Unit Size: 24 beds
Patient Ratio: 1:6-8
Job Description
- Provide nursing care in an acute rehabilitation setting.
- Work with patients recovering post-acute illness or injury.
Required Qualifications
- Valid RN license in Indiana
- Minimum of 1 year of rehabilitation nursing experience within the last year
- BLS certification issued by the American Heart Association
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Onboarding & Compliance Requirements

- Resume/Profile
- Two professional references
- Driver’s License or State ID (valid, not expired, matching address requirements)
Required Prior to Start (must be completed and/or updated as specified)
- Signed Assignment Agreement (offer letter and confirmations)
- Ascension EKG Exam (minimum 80%, max 2 attempts; updated every 3 years)
- Ascension Medical Surgical Exam (minimum 80%, max 2 attempts; updated every 3 years)
- Ascension Pharmacology Exam (minimum 80%, max 2 attempts; updated every 3 years)
- Ascension Core Competencies (HIPAA, Joint Commission, OSHA, etc.; minimum 80%, max 2 attempts; updated every 3 years)
- Ascension Onboarding Education Attestation
- Ascension Nursing Contract Worker Onboarding (completed onsite)
- Ascension Badge Photo (forward-facing headshot, white or blue background, no filters)
- Ascension Skills Checklist (clinical, completed within 1 year, updated annually)
- Ascension Criminal Background Check (7-year search including SSN trace, OIG, National Sex Offender; updated annually or after >90-day break)
- Ascension EPLS check (updated within 90 days of start or after >90-day break)
- Ascension Education Verification (within 90 days of start; rerun if >90-day break)
- Ascension Confirmation of Assignment (initial and updated with extensions)
- Ascension Employment Verification (past 7 years; updated annually or after >90-day break)
Health & Immunization Requirements (must be completed prior to start and updated as required)
- MMR: Proof of two vaccines or positive titer; no declinations accepted.
- Varicella: Proof of two vaccines, immunity, or history verified by provider; no declinations accepted.
- Tdap: Valid within 10 years; no declinations accepted.
- Hepatitis B: Vaccination series completion or signed declination.
- Influenza Vaccine: Optional; if declined during flu season, mask required.
- 2-Step PPD or IGRA (Quantiferon/Tspot): Recent testing required; if positive, chest X-ray and TB questionnaire required.
- Positive PPD requires negative chest X-ray within 1 year and annual TB questionnaire.
- Fit Test: Required for all travelers; preferred before start, can be done onsite; updated annually.
- Health Statement or physical exam confirming good health prior to first assignment; updated annually.
- 12 Panel Drug Screen: Within 30 days prior to start; updated annually; retesting required after >90-day break.

- Hiring Manager Evaluation and Name Change Documentation are optional post-assignment or as needed.
