Description

Registered Nurse (RN) Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 08:00 AM – 04:30 PM


Job Summary

As a Home Health RN, you will:

  • Assess patient conditions and implement care plans.
  • Provide ongoing education and support to patients and their families.
  • Perform admission processes and case management duties.
  • Deliver patient care aligned with community health practice standards.
  • Utilize thorough and timely electronic documentation and interdisciplinary communication.
  • Travel to patient cases as assigned.
  • Participate in weekend rotations approximately once every 6 weeks.

Required Qualifications

  • Valid RN license in Massachusetts.
  • Home Health experience within the last year.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Case Management experience.

Required Credentials & Compliance (Must be completed prior to or at onboarding)

  • Professional license verification with primary source validation (NURSYS and Board of Nursing required).
  • Submission of a recent color photo (within 6 months) meeting specified guidelines (no selfies, no filters, plain background).
  • Motor Vehicle Record (MVR) check within 45 days of start.
  • N95 Respirator Fit Test within 1 year, updated annually (no exemptions allowed).
  • Respirator Medical Clearance attestation if fit test required.
  • Documentation of immunity or vaccination for Measles, Mumps, and Rubella (MMR).
  • TB screening documentation within 3 months of start (skin test or blood test), updated if break >6 months.
  • Physician-signed health statement within 1 year, updated if break >6 months.
  • Criminal background check within 45 days prior to start, including SSN trace and national sex offender registry check, updated as required.
  • Massachusetts CORI background check within 30 days prior to start.
  • Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists within 30 days prior to start and annually.
  • Nurse Aide Registry check within 30 days of start (required for all employees in MA).
  • Verification of highest education level achieved.
  • Submission of a detailed resume covering the past 7 years, including city and state for all work and education.
  • Two professional references within the last 4 years, including at least one supervisor or higher.
  • Completion of a skills checklist within 1 year of submission.
  • Confirmation of Assignment/POI form upon acceptance and for any extensions.
  • Candidate submission coversheet for initial assignment submissions.
  • Beeline attestation completed after registration.

Additional Information

  • Weekend rotations occur approximately once every 6 weeks.
  • No religious or medical exemptions allowed for N95 fit testing.
  • Fit testing cannot be performed onsite; must be completed prior to start.
  • Positive TB test requires documentation of a negative chest x-ray (optional but recommended).
  • Name change documentation required if legal name differs from credential documents.

This position requires strong clinical skills in home health nursing, case management experience, and the ability to work independently while traveling to patient homes. Compliance with all credentialing and health requirements is mandatory.
