Overview

• Location: Newton, Massachusetts
• Employment type: LTA
• Contract length: 13 weeks
• Shift: 08:00 - 16:30
• Weekend rotations are once every 6 weeks
• Ability to travel to cases as assigned

Required Qualifications

• Valid RN license in Massachusetts
• Associate of Science in Nursing / Associate Degree in Nursing
• Home Health experience within the last year
• 1 year of Certified Home Care experience
• Case Management experience
• License/Certificate Verification is required

Role Responsibilities

• Assess patient conditions
• Implement care plans
• Provide ongoing education and support to patients and their families
• Perform admission processes
• Perform case management responsibilities
• Use timely electronic documentation and interdisciplinary communications
