Job Opportunity: RN Step Down - Travel Position

Required Credentials and Certifications

• BLS (Basic Life Support)
  American Heart Association only. Required for clinical positions.

• ACLS (Advanced Cardiovascular Life Support)
  American Heart Association only.

• NIHSS (National Institutes of Health Stroke Scale)

• License/Certificate Verification
  Primary source verification of a valid permanent RN license required. Temporary licenses not accepted. Must be updated every 2 years with RN license renewal.

Health and Immunization Requirements

• COVID-19 Vaccine
  Proof of vaccine series or approved medical/religious exemption preferred but not required.

• MMR (Measles, Mumps, Rubella)
  Positive titers within last 15 years required. Booster protocol applies if low titer.

• Varicella
  Positive titers within last 15 years required. Booster protocol applies if low titer.

• Hepatitis B
  Proof of full series, partial series with signed declination, or positive titers within 15 years.

• Tdap (Tetanus, Diphtheria, Pertussis)
  Proof of vaccination within past 10 years required. Declinations not accepted.

• Influenza Vaccine
  Required during flu season (Oct-Mar) or approved medical/religious exemption.

• 2 Step PPD OR IGRA (Quantiferon/Tspot)
  Negative test within last 6 months required. Annual requirement. If positive, clear chest x-ray and current TB questionnaire required.

• Positive PPD
  If positive, chest x-ray within 5 years and current TB questionnaire required.

• Fit Test
  Completed within 1 year of start; specific approved respirators listed.

• Physical/Health Statement
  Pre-employment physical exam within 1 year stating ability to work without restrictions.

Professional and Employment Verification

• UHS I-9 Attestation
  Proof of eligibility to work in the U.S.

• UHS Ohio Fingerprints
  Ohio BCI&I check and FCRA-compliant national criminal background check required. Attestation allowed if fingerprints pending.

• UHS Criminal Background Check
  Completed within 6 months prior to start, including SSN trace, county searches, and national criminal sweep.

• UHS Social Security Trace
  To be completed within 1 year if not included in background check.

• UHS NSO (National Sex Offender) Check
  To be completed within 30 days if not included in background check.

• UHS National Database Criminal Background Check
  Required for Ohio residents to meet fingerprint requirements.

• UHS OIG Search
  Completed within 6 months prior to start.

• UHS SAM/EPLS/GSA/OFAC Checks
  Completed within 6 months prior to start.

• UHS Education Verification
  Highest level of education verified. International education requires equivalency documentation.

• UHS Employment Verification
  Verification of employment within last 5 years through third-party verification.

Competency and Training Requirements

• UHS Competency Exam
  Must score at least 80%. Completed within 1 year prior to start.

• UHS Core Competencies
  Includes HIPAA, Joint Commission-NPSG, OSHA topics. 80% passing score required. Completed within 1 year prior to start.

• UHS Skills Checklist
  Clinical skill-specific checklist completed within 1 year prior to start (initial hire only).

• UHS Module Completion Certificate
  Completion of safety and compliance training modules prior to clearance.

Additional Requirements

• Drug Screen
  10-panel drug screen completed within 60 days prior to start. Instant drug screens not accepted.

• Driver's License/State ID
  Non-expired photo ID required.

• UHS Submission Packet
  Resume including education and work history, 2 professional references within last 2 years, and completed job application.

• UHS Badge Photo
  Professional headshot required.
