Requirements/Qualifications

• Valid RN license in California
• Associate of Science in Nursing / Associate Degree in Nursing
• Minimum 1 year of ICU (Intensive Care Unit) experience, including experience within the last year
• ECMO Specialist training and experience between 6 months and 1 year

Required Credentials and Certifications

1. BLS (Basic Life Support)

   • AHA or ARC Only
   • Required for onboarding

2. COVID-19 Vaccine

   • Proof of vaccine and 1 booster or exemptions must be submitted for approval.
   • Optional for onboarding.

3. MMR (Measles, Mumps, Rubella)

   • Proof of 2 immunizations, positive IGG titer, or booster if negative titer.
   • Required for onboarding.

4. Hepatitis B

   • Proof of 3 immunizations and positive antibody titer, or booster if negative titer.
   • Required for onboarding.

5. Influenza Vaccine

   • Required from October - March (unless otherwise indicated by CDPH).
   • Medical/Religious exemptions will be sent for approval.
   • Required for onboarding.

6. TB Test

   • Must be within 30 days of start. Negative results required.
   • Required for onboarding.

7. Health Statement

   • Statement signed and dated within 1 year of start stating traveler is able to work without restrictions.
   • Required for onboarding.

8. Driver's License/State ID

   • Required for onboarding.

9. Varicella

   • Proof of 2 immunizations, positive titer, or Declination accepted.
   • Required for onboarding.

10. Tdap (Tetanus, Diphtheria, Pertussis)

    • Proof of vaccination within past 10 years or Declination accepted.
    • Required for onboarding.

11. Covid Test

    • Required on start date for clinical staff.
    • Optional for onboarding.

12. Professional References

    • 2 Professional References, 1 minimum Supervisory.
    • Required for submission.

13. Criminal Background Check

    • Must be within 30 days prior to start.
    • Required for onboarding.

14. Drug Screen

    • Must be within 30 days of start, 10 panel standard test.
    • Required for onboarding.

15. License/Certificate Verification

    • Primary Source Verification of professional license, Nursys, and/or certification.
    • Required for onboarding.

16. Education Verification

    • A diploma, degree, or transcripts is acceptable.
    • Required for onboarding.

Additional Information

• Skill Specific Requirements: Clinical only; skill specific required to be completed within 1 year of start.
• HLC Testing: To be completed by Friday before start. Confirmation required for stipend.
• Documentation: Must be updated with each extension and retained by staffing agency.
