Job Description

  • Position: Registered Nurse (RN) specializing in Behavioral Health
  • Facility: Located in Wichita, KS
  • Employer: Part of a large hospital system

Required Credentials & Onboarding Requirements

Submission Requirements (Must be provided before submission)

  • Current nursing license with primary source verification within 30 days prior to start
  • Nursys verification within 30 days prior to start
  • Resume or professional profile
  • Two professional references completed prior to first assignment
  • Valid Driver's License or State ID (must not be expired and address must match requirements)

Onboarding Requirements (Must be completed prior to or at start)

  • Signed Assignment Agreement
  • Ascension Cerner Test-Out (Cerner test results and training checklist with signed attestation)
  • Basic Life Support (BLS) certification (AHA required)
  • Ascension Color Vision test (completed prior to start, updated every 3 years)
  • Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, max 2 attempts)
  • Ascension EKG Exam (passing score of at least 80%, max 2 attempts)
  • Nursing Contract Worker Onboarding (done onsite)
  • Pre-hire education attestations and modules (including Nova StatStrip)
  • CPI/MOAB High Risk Online Portion (Nonviolent Crisis Intervention or MOAB De-Escalation Course completed prior to start)
  • Documentation of immunizations and health screenings:
    • MMR (two vaccines or positive titer; no declinations accepted)
    • Varicella (two vaccines, serological evidence, or verified history; no declinations accepted)
    • Tdap (valid within 10 years; no declinations accepted)
    • Influenza vaccine (flu season Oct 1 - Mar 31; exemptions accepted with mask requirement)
    • Hepatitis B vaccination series completion or signed declination
    • Fit Test for respiratory protection (preferred prior to start, can be onsite)
    • 2 Step PPD or IGRA testing for TB (with annual TB questionnaire)
    • Positive PPD requires negative chest X-ray within 1 year and TB questionnaire
  • Health Statement or completed physical exam prior to first assignment, updated annually
  • Ascension Criminal Background Check (including SSN trace, employment verification, OIG search, national sex offender check; updated annually or after break in service)
  • EPLS check within 90 days of start
  • Ascension Skills Checklist (clinical specialty skills; completed within one year and updated annually)
  • 12 Panel Drug Screen within 30 days prior to start, updated annually or after break in service
  • Education verification relevant to clinical work (within 90 days of start, rerun if break in service)
  • Confirmation of Assignment documentation (initial and updates upon extension)
  • Employment verification for past 7 years (updated annually or after break in service)

Optional Credentials

  • Ascension Unit Specific Checklist (optional for onboarding, uploaded by unit leader)
  • CPI/MOAB High Risk On-Site Portion (completed at orientation)
  • Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire (if applicable)
  • Hiring Manager Evaluation (post-assignment)
  • Legal Name Change Documentation
