Travel RN - Case Management

Location: Wichita, Kansas
Employment Type: Travel

Unit Information

Unit: Case Management RN (917)
Setting: CPCU (Cardiac Progressive Care Unit, 3rd floor) and PCU (Progressive Care Unit, 4th floor - step down units)
Beds: CPCU - 14 beds, PCU - 14 beds
Patient Ratios: 1:14
Shift: Day Shift, Monday to Friday, 8:00 AM - 4:30 PM
EMR Systems: Meditech and Midas

Job Requirements

Specialty: Registered Nurse (RN) in Case Management
Required Experience: Minimum 2 years in specialty
Certifications: BLS
Licenses: Registered Nurse License
Preferred Experience:
- HCA hospital experience
- Midas and HCA case management experience
Must Have:
- RN or Licensed Social Worker
Prior Travel Experience: Not explicitly required

Job Description

Perform case management assessments
Discharge planning
Work in a fast-paced, busy, and constantly changing environment
Collaborate with a welcoming and supportive case management team
Work closely with consistent physicians and mid-level providers
CPCU focuses on cardiac step-down patients including CABG and TAVR patients
PCU manages a variety of medical step-down patients
Some patients require post-acute placements, many go home with cardiac rehab
