Job Description
As an RN in Case Management, you will provide individualized, holistic care to patients and their families, guiding them toward their health goals. Responsibilities include:
- Conducting comprehensive nursing assessments and documenting patient and family needs.
- Developing, implementing, and evaluating patient care plans in collaboration with patients, families, and healthcare teams.
- Managing complex or high-risk clinical situations by seeking additional resources as needed.
- Advocating for patients and families to ensure quality and satisfying care.
- Educating patients and families through individualized teaching plans, assessing learning needs and barriers.
- Communicating effectively with interdisciplinary teams to coordinate and optimize patient care.
- Delegating tasks appropriately within the healthcare team’s scope of practice.
Requirements
- Licensure: Valid California RN license
- Certifications: CPR certification
- Experience: Minimum of 1 year of experience in RN Case Management (based on typical expectations for this specialty; confirm with recruiter if needed)
- Contract Length: 13 weeks (approximately 3 months)
- Prior Travel Experience: Not explicitly required
