Job Opportunity: RN (Registered Nurse) - Care Coordination (Case Management)
Location: Sacramento, California

Job Description
Seeking an experienced Core Coordinator RN (case manager) for a travel contract. The ideal candidate will have:
- Minimum 3 years of hospital experience, including acute level care coordination
- Proficiency with Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs
- Experience with discharge planning and utilization review
- Active BLS certification and a valid California nursing license
- Prior CA Dignity/CommonSpirit acute care experience is required
Requirements
- Years of Experience: Minimum 3 years hospital experience
- Prior Travel Experience: Not explicitly required
- Certifications: Active BLS and CA RN license
- References: 2 supervisory references required (1 within 12 months, 1 within 3 years)
- Employment Restrictions: Candidates must not have been employed (full-time, part-time, or PRN) by any CommonSpirit-associated facility within the past year, except PRN through outside firms
Work Details
- Shift: 8-hour shifts preferred; 12-hour night shifts possible if candidate has ED background and is willing to work nights
- Overtime: Not allowed
- Patient Ratio: 1:20
- Floating: Must be able to cover all adult hospital areas; no coverage of women’s health without experience
- Orientation: 4 days webinar on charting plus 2 days preceptor
- Dress Code: Business casual conservative; no open-toed shoes or lab coats; minimal to no perfume; clean, professional appearance
- Additional Expectations:
- Fast learner, adaptable, flexible
- Excellent attendance required
- Must take lunches and breaks
- Weekend shifts and floating as needed
- Support: 24/7 Respiratory Therapy and Pharmacy available
