Job Opportunity: RN (Registered Nurse) - Care Coordination/Case Management
Location: Carmichael, California
Contract Length: 13 weeks
Position Summary
Seeking an experienced Care Coordinator for a travel contract role. The candidate will be responsible for both discharge planning and utilization management within a hospital setting.
Requirements
- Specialty: Care Coordination/Case Management
- Experience: Minimum 2 years in specialty and hospital experience (higher value confirmed)
- Certifications: Active BLS certification and California nursing license
- Skills:
- Experience with discharge planning and utilization management required
- Interqual experience highly preferred (training provided if needed)
- Ability to handle all payers including uninsured and homeless populations
- Strong collaboration, teamwork, effective communication, and ability to reprioritize tasks throughout the day
- Prior Travel Experience: Not explicitly required
- Additional:
- Must have prior experience working within CA Dignity Health system (required for consideration)
- Candidates cannot have been employed by CommonSpirit-associated facilities within the past year (exceptions for PRN through outside firms)
- Two supervisory references required (one within 12 months, one within 3 years)
Work Details
- Unit: Case Management/Care Coordination
- Patient Load: 15-20 patients/consults per day
- Shift: Typically 8 AM to 5 PM, varies by department
- Weekend Requirement: Every other weekend (at least one full weekend per month)
- Holidays: Not required but may work if willing
- Uniform: Business casual attire
- Hospital Coverage: All hospital areas except outpatient and home care
- Orientation: 5 days new employee care coordination orientation; unit orientation varies
- Support: Hospitalist and intensivist on-site; respiratory therapy 24/7; pharmacy not 24/7
Duties & Responsibilities
- Conduct initial assessments
- Perform discharge planning and utilization review
- Use Interqual criteria and Cerner EMR system
- Manage cases involving diverse payer sources and social situations
- Collaborate effectively with multidisciplinary teams
- Float as needed within the department
