Job Opportunity: Interim Leader - Case Management

Location: Vicksburg, Mississippi
Employment Type: Interim Leader
Contract Length: 12 weeks (approximately 3 months)

Job Summary

The Interim Care Management Director is responsible for overseeing the implementation, development, and management of Care Management and Resource Management programs. This role ensures compliance with regulatory requirements, establishes efficient patient care and discharge planning processes, and provides leadership to the Care Management team. The Director collaborates with staff, physicians, and community resources to deliver patient-centered care and achieve optimal outcomes.


Key Responsibilities

  • Oversee and maintain an effective, compliant Care Management program meeting regulatory and accreditation standards.
  • Lead and direct staff, ensuring accountability and adherence to scope of practice.
  • Conduct chart reviews to evaluate severity of illness and intensity of services for admissions, continued stays, and discharge planning.
  • Refer cases not meeting criteria to Physician Advisor or Utilization Management Committee and ensure follow-up.
  • Identify avoidable days and implement communication systems to resolve them.
  • Coordinate discharge planning and social services to ensure compliance with federal/state laws and accreditation standards.
  • Act as liaison to community agencies for post-hospital placement and support.
  • Ensure accurate and timely reporting of mandated CPS/APS cases and educate staff on reporting requirements.
  • Participate in Patient Care Conferences and Length of Stay meetings to support efficient care delivery.
  • Provide leadership, mentorship, and professional development for departmental staff.
  • Schedule employees and consult on staffing issues.
  • Conduct performance evaluations and provide feedback.
  • Collaborate with hospital leadership on strategic planning, budgeting, and resource allocation.
  • Monitor expenditures and implement cost-effective service delivery.
  • Evaluate and implement new technologies to improve operational efficiency.
  • Develop and enforce departmental policies, procedures, and protocols.
  • Ensure compliance with regulatory bodies and participate in audits and accreditation processes.
  • Foster collaboration and communication across leadership teams and stakeholders.
  • May perform staff role duties as needed, meeting all competency requirements.

Required Qualifications

  • Bachelor's Degree in a relevant field with 3-5 years of experience in a closely related field, or seven (7) plus years of direct experience.
  • Minimum of 7 years of direct experience in the field.
  • At least 3 years of experience in a closely related field.
  • Minimum of 1 year of management experience within the last year.

Licenses and Certifications

  • Registered Nurse (RN) license required (State Licensure and/or Compact State Licensure).
  • Basic Life Support (BCLS) certification required within seven (7) days of hire or transfer.
