Care Manager (Travel)
Location: Portland, Maine
Contract Length: 13 weeks
Shift: 1st shift
Hours per Week: 36 (occasional weekend hours available but not required)
Call Requirements: No
Float Requirements: Yes, must be able to cover Care Management department needs
Job Description
- Responsible for discharge planning and care coordination
- Patient ratio approximately 1:20-24
- Work within Care Management department
Required Qualifications
- Valid RN license in Maine
- Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
- Minimum 5 years of Case Management experience, including experience within the last year
- Experience in discharge planning
- Experience in care coordination
- Experience as a Care Manager
