Description
Registered Nurse (RN) - Home Health / Hospice
Location: Beaver Dam, Wisconsin
Contract Length: 13 weeks
Shift: Days (Monday through Friday, 8:00 AM – 4:30 PM)
Hours per Week: 40 hours
Job Overview
- Provide home health and hospice nursing care primarily for a geriatric population.
- Patient diagnoses include heart disease, chronic respiratory diseases (CHF, COPD), pneumonia, dementia/Alzheimer’s, depression, stroke, cancer, kidney disease, diabetes, liver disease, wounds, joint replacement, surgery recovery, and medication management.
- Patient load: 5-6 patients per day, approximately 25-30 points per week.
- Visits include routine and OASIS assessments.
- No pediatric patients.
- Care involves both home health and hospice visits.
- No floating to other departments.
- On-call requirement: 1 week every 6 weeks (weekend on-call included, but on-call days do not have to be consecutive).
- Must be available to work at least one major holiday and one minor holiday.
- Weekend work only required during on-call shifts.
Required Qualifications
- Current RN license.
- BLS (AHA) certification required.
- Experience in home health and hospice nursing.
- Skills in critical thinking, chronic disease management, patient and family education.
- Experience with NPWT (Negative Pressure Wound Therapy), ostomy care, central line care, IV administration, and complex wound care.
- Hospice experience including pain and symptom management, end-of-life care, empathy, and compassion.
- Prior travel experience required.
Work Environment & Additional Information
- Department: Home Health / Hospice.
- No ventilator patients; occasional CPAP and BiPAP patients who self-manage.
- Certified nursing assistants available to support.
- Daily 15-minute huddle calls, weekly interdisciplinary team meetings, and monthly all-staff meetings.
- Dress code: Solid color scrubs (no patterns or logos unless company-issued), closed-toe non-slip shoes with socks or hosiery; no flip-flops, open-toe shoes, or sandals allowed.
- Orientation includes 1 day of computer-based training and 2-5 days of field training/shadowing depending on experience.
- Clinicians must clock in and out at the facility.
- Administrator available after hours, weekends, and holidays for support.

- Candidates living within 80 miles of the facility may have adjusted rates (not included here).
- Clinicians who have previously worked for this employer must have ended their prior employment at least 6 months before reapplying.
