Registered Nurse (Medical/Surgical) - New York, Ithaca
Job Summary
- Provide direct patient and family support and nursing care within full scope of practice.
- Participate in development, implementation, monitoring, and evaluation of patient and community programs and services.
- Responsibilities include observation, data collection, planning, implementation, and evaluation of patients’ physical, psychological, emotional, educational, and spiritual needs.
- Collaborate as a member of the interdisciplinary team.
Employment Details
- Employment Type: Long-Term Assignment (LTA)
- Contract Length: 8 weeks
- Shift: 6:45 PM – 7:15 AM (overnight)
Required Qualifications
- Valid RN license in New York
- Associate of Science in Nursing / Associate Degree in Nursing
- Minimum 2 years of Medical/Surgical nursing experience
- Medical/Surgical nursing experience within the last year
- BLS certification issued by the American Heart Association
- PALS certification issued by the American Heart Association
Required Onboarding Documentation and Compliance
- Central skills checklist for the assigned unit, updated annually.
- Core competency exams (Nursing Core Comp I, II, and III) with a minimum score of 80%.
- Rhythm Interpretation Exam.
- COVID-19 vaccine proof or agency declination (optional).
- Immunizations and health requirements:
- MMR: Positive IgG titer or 2 vaccine series; if negative/equivocal, vaccination required within 30 days.
- Varicella: Positive IgG titer or 2 vaccine series; if negative/equivocal, vaccination required within 30 days.
- Hepatitis B: Positive antibody titer, 3 vaccine series, or declination form.
- TB Test: One of the following within required timeframes:
- Two PPD skin tests (one within 12 months, one within 3 months), or
- One Quantiferon/Spot test within 12 months.
- For past positives, chest x-ray within 12 months may be required.
- Tdap: Preferred within last 10 years; declination form accepted if not available.
- Fit Test: Must be completed prior to start and within last 12 months; annual requirement.
- Physical/Health Statement: Within last 12 months, including vitals, height, weight, Snellen eye chart, and physician’s statement of clearance without restrictions.
- Drug Screen: 9+ panel drug screen within 30 days of start.
- Background Check: Must be run within 30 days of start, including SSN address trace, county and national criminal searches, FACIS 3 check, and all aliases.
- Valid US Driver’s License or State ID (not expired).
- Color headshot photo (professional, no filters, JPEG format).
- Employment verification of most recent employer.
- Two professional references: one supervisor reference from last 12 months, one supervisor or peer reference from last 3 years.
- Signed and dated job description.
- Completion of agency-specific forms and acknowledgments including code of conduct, sexual harassment acknowledgement, parking registration, confirmation of assignment, self-study packet and quiz.
- Central Epic Questionnaire and TB Questionnaire forms (agency-specific).
Additional Notes
- All required documentation and certifications must be current and valid throughout the assignment.
- Annual requirements include fit test, physical/health statement, and TB testing.
- Vaccination series may be allowed to be completed within 30 days of assignment start if initial dose is documented.
- This position involves overnight shifts and requires collaboration within an interdisciplinary team.
