Registered Nurse - Case Management (Travel)

Job Description
- Coordinate management of patient care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
- Provide comprehensive assessment, care planning, implementation, and evaluation of individual patient needs.
- Enhance quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness.
- Integrate case management, utilization review, and discharge planning functions.
- Serve as a patient advocate addressing holistic patient needs.
- Collaborate strongly with interdisciplinary teams (IDT).
- Work schedule: Monday to Friday, 8 AM to 5 PM.
Required Qualifications
- Valid RN license in California
- Minimum 5 years of Case Management experience, including experience within the last year
- Associate of Science in Nursing or Associate Degree in Nursing
- Experience in a Long Term Acute Care (LTAC) setting
- Acute Hospital Case Management experience
- Proficient with MCG (Milliman Care Guidelines)
- Experience with Utilization Management and Utilization Review (InterQual)
- Skilled in Discharge Planning
- American Heart Association BLS certification
