    Description

    Registered Nurse - Acute Rehabilitation Unit (Travel/Contract)

    Location: Crescent City, California
    Shift: Night (3 shifts per week, 12 hours each, 7:00 PM - 7:00 AM)
    Contract Length: 13 weeks


    Job Overview

    • Provide nursing care on a 10-bed Acute Rehabilitation Unit.
    • Manage patient and family experience using critical thinking, teamwork, communication, and nursing judgment.
    • Care for patients primarily adults to geriatrics, with occasional pediatric to geriatric patients.
    • Float to Medical Care Unit (MCU) and Telemetry (Tele) departments as needed.
    • Responsible for comprehensive nursing care from admission to discharge, including directing ancillary staff.
    • Utilize nursing process: assessment, planning, implementation, and evaluation to meet physiological, psychosocial, safety, learning, and self-care needs.

    Required Qualifications

    • Valid Registered Nurse (RN) license in California
    • Minimum 1 year of experience in a Rehabilitation setting, with experience within the last year
    • Basic Life Support (BLS) certification issued by American Heart Association (AHA) or American Red Cross (ARC)
    • Pediatric Advanced Life Support (PALS) certification issued by American Heart Association (AHA) or American Red Cross (ARC)
    • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

    Background and Screening Requirements

    • 7-year background check including SSN trace with all aliases, county searches, nationwide sex offender search, education verification, and FACIS Level 3 screening.
    • EPLS/GSA/SAM, HHS/OIG, OFAC Sanctions List, and Medi-Cal Suspended and Ineligible Provider List checks as applicable.
    • Education verification required unless highest education is high school; Bachelor of Science in Nursing preferred.
    • Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to CA law).
    • Employment history verification covering 7 years with no gaps over 30 days; must include city and state of employment.
    • References: 2 professional references within 3 years required (Aya requirement, not client).

    Health and Immunization Requirements

    • Physical exam within 1 year of start, cleared for work.
    • Fit mask test within 1 year, renewed annually (specific mask types required).
    • Immunizations and/or titers required:
      • Tetanus/Diphtheria/Tdap (adult TDAP within 10 years or lifetime TDAP with booster)
      • Negative TB test (QGold or TSPOT within 90 days or 2-step PPD) renewed annually
      • Measles (Rubeola), Mumps, Rubella: Positive IGG titer or vaccination series with boosters as needed
      • Hepatitis B: Positive titer, series, or signed declination
      • Varicella: Positive IGG titer or vaccination series with boosters as needed
      • Seasonal Flu vaccination or signed declination during flu season (Nov 1 - Apr 30)
    • Drug testing: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids).

    Competency and Skills

    • Unit-specific skills checklist required, renewed annually.
    • Medication/pharmacology competency exam as applicable.
    • Dysrhythmia/EKG competency exam may be required for Telemetry units.

    Facility-Specific Requirements

    • Review and sign facility policies including Standards of Business Conduct, Vendor Training, Code of Conduct, Overpayment Refund Policy, and General Regulatory Orientation prior to start.
    • Complete TB Screening Questionnaire within 90 days of start, signed by candidate and healthcare provider (MD, PA, NP/APN, DO, or RN).
    • Complete Epic Questionnaire (RN only) for candidates starting 11/3/2025 and onward.
    • Background check attestation and conditional clearance policies apply; no conditional clearances allowed at certain facilities.
    • Religious exemption forms required for facial hair (PAPR) and vaccine exemptions; must be approved case-by-case.

    Additional Notes

    • Candidate must have full 7 years of work history documented in the application with no gaps.
    • Employment verification must be thorough; any gaps over 1 year require approval.
    • Candidates with positive TB history require a negative chest x-ray within 1 year of start.
    • Failed fit tests due to facial hair require shaving and retesting; other failures require PAPR training.
    • Candidates must comply with all immunization and screening requirements or provide approved medical/religious exemptions.

    This position offers an opportunity to work in a specialized acute rehabilitation setting with a focus on adult and geriatric patient care, requiring strong clinical skills and flexibility to float to related units.
