    Job Opportunity: Case Manager

    Location: Sacramento, California
    Employment Type: Travel/Contract
    Contract Length: 13 weeks
    Shift: Day shift (Monday to Friday, 8:00 AM – 4:30 PM)
    Hours: 40 hours per week (5 days x 8 hours)


    Job Description

    • Work in an adult stem cell transplant outpatient clinic setting.
    • Responsibilities include managing authorizations, handling new patient referrals, collaborating with multiple departments to schedule appointments.
    • Respond to patient messages via the electronic medical record system.
    • Patient population includes diagnoses such as leukemia, lymphoma, multiple myeloma, and stem cell transplant recipients.

    Required Qualifications

    • Valid RN license in California
    • Minimum 1 year of Case Management experience within the last year
    • Associate of Science in Nursing / Associate Degree in Nursing
    • Oncology experience
    • Basic Life Support (BLS) certification issued by the American Heart Association

    Compliance and Documentation Requirements

    • Valid driver’s license/state ID (clear photo required).
    • Criminal background check within 30 days of start (7-year search including SSN trace, all counties/states lived/worked, sex offender and nationwide criminal search).
    • E-Verify compliance required.
    • Negative 10-panel drug test within 30 days of start.
    • Physical exam clearance within one year of start.
    • Fit testing for N95 masks within one year of start (onsite fit testing possible if prior attempts fail).
    • Immunizations and titers required:
      • Tetanus/Diphtheria/Tdap (within 10 years, no declinations accepted).
      • Tuberculosis screening (Qgold or TSPOT within 90 days or 2-step PPD).
      • Measles, Mumps, Rubella (positive titer or two vaccines; boosters required if negative/equivocal).
      • Hepatitis B titer required; declination or series accepted with conditions.
      • Varicella (positive titer or two vaccines; boosters required if negative/equivocal).
      • Seasonal flu vaccine or declination required by November 1.
    • Signed confidentiality statement and HIPAA training confirmation prior to start.
    • Completion of facility-specific orientation modules and compliance training before start date.
    • Employment application covering last 3 years with explanations for gaps over 6 months.
    • Two clinical references required, including one from the most recent or recent employer within 6 months.
    • Unit-specific competency exams and skills checklist required prior to start.
    • Badge pickup required prior to orientation during business hours.

    • This position is outpatient oncology-focused with emphasis on stem cell transplant patient management.
    • Candidates must be prepared to complete all required training and documentation in a timely manner to avoid delays or cancellation of start date.
    • The facility enforces strict compliance with immunization, background checks, and competency requirements.
    • Badge pickup location requires a non-expired photo ID and must be done during specified hours before orientation.
