Job Opportunity: Outpatient Registered Nurse Care Manager
Location: Napa, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Work in a licensed Adult Day Health program serving frail elderly, younger disabled, and people with Alzheimer's Disease and related dementia.
- Program licensed for 100 patients/day; average daily census ~72.
- Role involves chronic disease case management, monitoring medical conditions alongside interdisciplinary team (IDT) and primary care providers (PCP) in the community.
- Provide caregiver education and support.
- Goal: reduce emergency department visits and hospitalizations, prevent premature nursing home placement.
- Use Turbo TAR EMR (specific to California Adult Day Health Care industry).
- Patient ratio approximately 1 RN to 40 patients daily.
- Facility type: Outpatient Clinic.
- Attire: Any basic color scrubs.
- Parking: Free.
- Mileage reimbursement per IRS standard.
Schedule
- Shift: Day shift, 5 days per week, 8 hours per day (8:00 AM – 5:00 PM).
- No weekend requirement.
- No on-call duties.
- No floating outside scope of practice.
- No block scheduling accommodations.
Required Qualifications
- Valid Registered Nurse (RN) license in California.
- Minimum of 1 year of case management experience, with case management experience within the last year required.
- Clinic experience preferred.
- Basic Life Support (BLS) certification issued by the American Heart Association or Canadian Heart and Stroke Foundation.
- Pediatric Advanced Life Support (PALS) certification issued by the American Heart Association or Canadian Heart and Stroke Foundation.
- NIH Stroke Scale (NIHSS) certification.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Patient Care & Documentation
- Charting in Turbo TAR EMR system.
- Daily patient ratio: 1:40.
Compliance & Background Requirements
- Background check within 90 days of start, covering all locations lived/worked in past 7 years; must be updated annually.
- Education verification required for nursing degree; international education requires additional documentation.
- Drug screening: 9-panel drug screen within 12 months of start.
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating ability to work with or without accommodation.
- Immunizations and health requirements:
- Tetanus/Diphtheria/Tdap (within 10 years; declination accepted).
- Negative TB test or chest X-ray within 12 months; annual updates required.
- Measles, Mumps, Rubella (2 MMR vaccines or positive titer; declination accepted).
- Varicella (2 vaccines or positive titer; declination accepted).
- Hepatitis B vaccination series or signed declination.
- Seasonal flu vaccine during flu season or declination.
- COVID vaccination record preferred; declinations not required but unvaccinated must follow infection prevention protocols.
- Respirator medical clearance required annually for travel candidates.
- Fit mask testing required for travel candidates, updated annually.
Certifications & Competencies
- BLS certification (AHA or Canadian Heart & Stroke Foundation).
- PALS certification (AHA or Canadian Heart & Stroke Foundation).
- NIH Stroke Scale (NIHSS) certification.
- Additional certifications (ACLS, NRP, Chemotherapy, EFM, IV Certification, CPI, Management of Assaultive Behavior) as applicable per clinical track and facility requirements.
- Medication/pharmacology competency exam as applicable.
- Competency exams required as applicable by specialty.
Documentation & Onboarding
- Employment application covering at least last 5 years of employment required.
- Minimum of 2 professional references covering 1 year of employment in a comparable specialty/setting required.
- Passport-style badge photo with neutral background required.
- Completion of annual regulatory and specialty-specific training modules via LMS (Workramp).
- EPIC training modules may be required depending on facility assignment.
Additional Notes
- Facility may call off traveler up to 1 shift per pay period (every 2 weeks).
- Confirm return-to-office (RTO) restrictions with hiring manager.
- No floating to other facilities expected but float policy acknowledgment required.
