Case Manager RN

Overview

• Location: Oakland, California
• Employment type: Travel/Contract
• Shift: Day, 5x8 hours

• Schedule: Every other weekend

Required Qualifications

• Valid RN license in California
• Case Management (5 years of experience)
• Case Management experience within the last year
• BLS issued by American Heart Association or American Red Cross
• Associate of Science in Nursing / Associate Degree in Nursing
• Experience in a(n) Inpatient setting
• Acute Hospital Case Management Experience

Role Summary

• Responsible for care coordination and care transitions planning throughout the acute care patient experience.
• Works with physicians, utilization management, social work, bedside nursing, and other interdisciplinary partners to support timely transitions to the appropriate level of care.
• Focuses on reducing unnecessary admissions or readmissions and coordinating resources across the continuum of care.

Key Responsibilities

• Complete patient assessment and identify readmission risk, psychosocial needs, and supportive service needs.
• Lead care coordination planning and support cost-effective, clinically appropriate transitions.
• Review documentation to confirm level of care requirements and support utilization management decisions.
• Monitor length of stay and address barriers that delay transition.
• Communicate and document transition plans, referrals, and follow-up needs.
• Support safe discharge planning and coordination with community resources and post-acute services.
• Participate in department operations and contribute to process improvement and professional communication.
