Case Management Discharge Planning - RN Days

Overview

  • Specialty: RN Case Manager
  • Location: Boston, Massachusetts
  • Employment type: LTA
  • Contract length: 13 weeks
  • Schedule: Day shift, Monday–Friday
  • Hours: 40 hours per week
  • Shift pattern: May be 4x10s or 5x8s, depending on operational need

Required Qualifications

  • Valid RN license in Massachusetts
  • Associate of Science in Nursing / Associate Degree in Nursing
  • Case Management (2 years of experience)
  • Case Management experience within the last year
  • Experience in an Acute Care setting
  • Discharge Planning
  • Acute Hospital Case Management Experience
  • MA Conditional Licenses are not accepted
  • NURSYS and BON verification are required for nursing licenses

Onboarding / Compliance Requirements

  • Fit test for N95
  • Respirator Medical Clearance attestation, if fit test is required for the role
  • MMR immunity or vaccination documentation
  • TB test documentation
  • Health statement signed by an MD, DO, NP, or PA
  • Criminal background check
  • Massachusetts CORI check
  • Resume including the past 7 years of work experience
  • 2 references within the past 4 years, including at least 1 supervisor or above in the skillset
  • Skills checklist completed within 1 year of submission
  • Confirmation of Assignment/POI form
  • Education verification
  • SSN trace
  • National sex offender check
  • Exclusion checks
  • Beeline attestation
