Case Manager
Overview
- Specialty: RN Case Manager
- Location: Allston, Massachusetts
- Employment type: LTA
- Shift: 08:00 - 16:30
- Contract length: 8 weeks
Role Summary
- Experienced case manager responsible for initial assessment, utilization review, leading and documenting ITC meetings, patient and family interactions, arranging skilled home care services, coordinating vendors for oxygen and respiratory equipment, enterals, TPN, and hospital beds, and participating in care plan meetings.
Required Qualifications
- Valid RN license in Massachusetts
- Case Management (1 year of experience)
- Case Management experience within the last year
- Associate of Science in Nursing / Associate Degree in Nursing
- Active nursing license or certificate verification is required for onboarding.
- Resume must include the past 7 years of work experience.
- Two references are required, including at least 1 supervisor or above in the skillset.
- Skills checklist must be completed within 1 year of submission.
- Education verification is required.
- Background checks and registry checks are required for onboarding, including criminal background check, Massachusetts CORI, SSN trace, national sex offender check, exclusion checks, and CNA registry checks for all states lived, worked, attended school, or held a license in.
- Fit testing and respirator medical clearance are required if the skill requires it.
- TB screening, MMR documentation, health statement, COVID declination form, and dementia training are required for onboarding.
- Orientation summary and confirmation of assignment/POI form are required for onboarding.
- Beeline attestation is required for new applicants.
Optional / Preferred
- BLS is optional.
- AHA is preferred for BLS; Red Cross is accepted.
