RN Outpatient Case Manager

Location

Glenwood Springs, Colorado

Required Qualifications

Valid RN license in Colorado
Case Management (3 years of experience)
Case Management experience within the last year
Associate of Science in Nursing / Associate Degree in Nursing
Experience with Epic

Role Summary

Coordinates inpatient and outpatient care in the overall admission process and access to healthcare

Housing

Potential housing available during the assignment, subject to availability
Newly remodeled studio and 1-bedroom apartments are available near the hospital
Housing includes Wi-Fi, utilities, on-site laundry, and furnishings
No deposit required

Resume listing hospital case management and Epic experience
Copy of Nursys or state RN license
Skills sheet

Additional Notes

Traveler’s permanent address must be at least 75 miles from the facility
CAPS registration is required and may take up to two weeks to obtain
