Home Care Registered Nurse

Job Overview

Required Qualifications

  • Valid RN license in Vermont
  • Associate of Science in Nursing / Associate Degree in Nursing
  • BLS
  • Home Health experience within the last year
  • 2 years of Home Health experience
  • Hospice experience
  • Experience with OASIS

Key Responsibilities

  • Assess patient and caregiver needs and develop a plan of care
  • Serve as case manager and manage caseloads and visit times
  • Complete OASIS documentation and other required forms accurately and on time
  • Implement nursing interventions and procedures
  • Provide patient and caregiver education
  • Coordinate care with other disciplines and providers
  • Maintain accurate nursing documentation
  • Coordinate safe discharges and transfers
  • Participate in peer review audits and case conferences
