Hospice Nurse

Job Overview

Location: Sunnyvale, California
Employment type: Contract
Contract length: 13 weeks
Schedule: Days, Sunday–Thursday; 10am–7pm or 11am–8pm
Hours per week: 40
Service area: San Francisco County, San Mateo County, and Santa Clara County

Role Summary

Admits hospice patients and assesses physical, psychosocial, environmental, safety, and developmental needs
Develops individualized plans of care and documents observations and interventions
Coordinates care with a multidisciplinary team
Participates in quality assessment and performance improvement activities
Conducts follow-up visits as needed

Required Qualifications

Valid RN license in California
Associate of Science in Nursing / Associate Degree in Nursing
Hospice experience
Case Management (1 year of experience)
Case Management experience within the last year
BLS
