Overview

• Travel/contract role in Oakland, California
• Day shift: 5x8s, 8:00–16:30
• 40 hours per week
• Every other weekend

Required Qualifications

• Valid RN license in California
• Case Management (5 years of experience)
• Case Management experience within the last year
• BLS issued by American Heart Association or American Red Cross
• Associate of Science in Nursing / Associate Degree in Nursing
• Experience in a(n) Acute Care setting
• Discharge planning experience
• Acute Hospital Case Management Experience
• Utilization Management

Role Summary

• Coordinates care and transitions across the acute care patient experience
• Works with physicians, utilization management, social work, bedside nursing, and other interdisciplinary partners to support timely discharge or transition to the appropriate level of care
• Focuses on resource coordination, utilization review, and reducing unnecessary admissions or readmissions
• May support complex clinical and social situations, including emergency department cases

Key Responsibilities

• Performs patient assessment and reviews medical, psychosocial, and financial information
• Leads care coordination planning and supports safe, cost-effective transitions
• Reviews documentation to confirm level of care requirements and supports utilization management
• Monitors length of stay and transition barriers
• Communicates and documents care plans, referrals, and transition needs
• Supports post-acute planning, follow-up needs, and regulatory compliance
• Participates in rounds and department operations
• Collaborates with patients, families, payers, community resources, and the care team
