Case Manager RN
Overview
- Travel/contract role in Oakland, California
- Day shift: 5x8s, 8:00–16:30
- 40 hours per week
- Every other weekend
Required Qualifications
- Valid RN license in California
- Case Management (5 years of experience)
- Case Management experience within the last year
- BLS issued by American Heart Association or American Red Cross
- Associate of Science in Nursing / Associate Degree in Nursing
- Experience in a(n) Acute Care setting
- Discharge planning experience
- Acute Hospital Case Management Experience
- Utilization Management
Role Summary
- Coordinates care and transitions across the acute care patient experience
- Works with physicians, utilization management, social work, bedside nursing, and other interdisciplinary partners to support timely discharge or transition to the appropriate level of care
- Focuses on resource coordination, utilization review, and reducing unnecessary admissions or readmissions
- May support complex clinical and social situations, including emergency department cases
Key Responsibilities
- Performs patient assessment and reviews medical, psychosocial, and financial information
- Leads care coordination planning and supports safe, cost-effective transitions
- Reviews documentation to confirm level of care requirements and supports utilization management
- Monitors length of stay and transition barriers
- Communicates and documents care plans, referrals, and transition needs
- Supports post-acute planning, follow-up needs, and regulatory compliance
- Participates in rounds and department operations
- Collaborates with patients, families, payers, community resources, and the care team
