Description

Case Management RN

Location

Fort Wayne, Indiana

Contract

13 weeks

Schedule

  • 8-hour day shifts
  • Monday through Friday
  • Weekend rotation
  • On-site

Required Qualifications

  • Valid RN license in Indiana
  • Associate of Science in Nursing / Associate Degree in Nursing
  • BLS
  • 2 years of Case Management experience
  • Case Management experience within the last year
  • Experience in an inpatient setting
  • Acute hospital case management experience
  • Competency with MCG and InterQual
  • Utilization review experience
  • Ability to drive stay progression and complete discharge planning in a manner that expedites patients transition to the next level of care
  • Utilization Review (InterQual)

Preferred

  • Certification in Case Management

  • Utilization review
  • Comprehensive patient assessment to support discharge planning
  • Stay progression and discharge planning
  • Collaboration with the care team
  • Documentation in Epic

Additional Details

  • May float to other units or floors while performing the same case management role
  • Uses MCG and InterQual
  • Local travelers within 50 miles are subject to a reduced rate
  • Client may call off or cancel up to 24 hours per 13-week assignment
  • 8 hours of non-billable orientation
  • Business casual dress code with white lab coat or eggplant-colored scrubs
  • Close-toed shoes required
