• 4 x 10-hour shifts per week
• No weekends or holidays

Contract Length

• 13 weeks

Required Qualifications

• Bachelor of Science in Nursing
• Valid RN license in California
• Current Basic Life Support certification issued by the American Heart Association
• 3 years of Case Management experience
• Case Management experience within the last year
• Oncology experience

Key Responsibilities

• Coordinates patient care and support across the continuum of care
• Collaborates with health care providers to ensure continuity of care and coordination of patient services
• Provides patient and family education and prepares patients/families for procedures and tests
• Develops educational materials, workshops, staff training, orientation, and conferences
• Assesses learning needs and evaluates staff competency
• Coordinates nursing staff activities and schedules physicians
• Develops and implements processes and procedures to improve resource use and patient care services
• Supports new program implementation and service promotion
• Serves as a liaison and consultant for providers, care settings, and care teams
• Applies performance improvement methodology and uses quality assurance and research to identify improvement opportunities
• May provide direct patient care
