Case Manager
Overview
- Travel/contract role in Burbank, California
- Start date: ASAP
- Day shift, 5x8 hours
- 40 hours per week
- Contract length: 26 weeks
- Acute care facility
Schedule and work expectations
- No floating expected
- Typically one full weekend every 4 weeks, depending on department need
- RTO limit: no more than 7 days
Required qualifications
- Valid RN license in California
- Associate of Science in Nursing / Associate Degree in Nursing
- 2 years of acute medical Care Coordination/Discharge Planning experience
- Acute Care Case Management (2 years of experience)
- Acute Care Case Management experience within the last year
