Home Health Case Manager RN
Role Overview
- Travel/Contract RN role in Sacramento, California.
- Responsible for overall patient management, including direct care with a designated visit productivity standard.
- Plans, organizes, and directs patient care services for the assigned caseload.
- Coordinates the interdisciplinary team to implement the plan of care and manage visit utilization and expenses.
- May assist with agency orientation, competency evaluations, and consultation to clinical staff.
Required Qualifications
- Valid RN license in California.
- Associate of Science in Nursing / Associate Degree in Nursing.
- Case Management experience within the last year.
- 2 years of Case Management experience.
- Experience in a Home Health setting.
- Discharge Planning.
- BLS - Basic Life Support issued by the American Heart Association or American Red Cross.
Assignment Details
- 13-week contract.
- Day shift: 8:00–16:30, 5x8 hours.
- 40 hours per week.
Additional Notes
- Motor Vehicle Report is required.
- Personal or auto insurance is required.
- 7-year background check is required within 6 months of start.
- 7 years of work history must be included in the application with no gaps.
- Unit-specific skills checklist is required.
- Full 7 years of work history must be completed in the application, including city and state of each location worked.
