Description
Case Manager
Overview
- Travel/contract role in Atlanta, Georgia
- Day shift: Monday–Friday, 8:00 AM–4:30 PM
- 40 hours per week
- No weekends
- No call
- Floating may be required within the facility and scope of practice
- Local candidates accepted
Role Focus
- Assess, evaluate, and support discharge planning
- Coordinate services to the patient’s appropriate level of care
- Monitor treatment progress and barriers to discharge
- Track length of stay and follow Medicare-related discharge guidelines
Required Qualifications
- Valid RN license in Georgia
- Case Management (3 years of experience)
- Case Management experience within the last year
- One completed travel assignment
- BLS - Basic Life Support issued by American Heart Association or American Red Cross
- Associate of Science in Nursing / Associate Degree in Nursing
Schedule Expectations
- Monday through Friday schedule
- No weekend shifts
- No call
- Up to 7 days of requested time off
- No time off requested during September
Compliance and Onboarding
- Full compliance is required before start
- Background check must be complete before start
- Education verification is required and may be pending at start with proof it is in progress
- Photo ID is required
- Skills checklist within 12 months is required and updated annually
- Most recent employer verification is required and may be pending at start with proof it is in progress
- 2 clinical references are required
- Physical exam within 12 months is required
- Negative TB screening or chest x-ray is required
- TB questionnaire is required
- Drug screen within 60 days of start is required
- Flu vaccination or declination is required
- Color blind testing is required
- Medication test/pharmacology competency exam is required for RNs
- Competency exam is required for nursing candidates
- Dysrhythmia/EKG competency exam is required for telemetry-related roles only
Additional Notes
- Holiday coverage may be required depending on assignment dates
- Non-billable orientation hours vary by contract length
