Description

RN - Case Manager

Overview

  • Coordinates care and services for patients needing assistance or meeting Care Management criteria.
  • Based in Billings, Montana.
  • Day shift, 8 hours per day, 40 hours per week.
  • Contract length: 13 weeks.

Required Qualifications

  • Valid RN license in Montana.
  • Bachelor of Science in Nursing.
  • 3 years of Case Management experience.
  • Case Management experience within the last year.
  • Experience in an Acute Care setting.
  • Hospital care management experience.
  • BLS.

Preferred Qualifications

  • 5 years of nursing experience.
  • ACM or CCM certification.

Key Responsibilities

  • Coordinates discharge planning.
  • Assesses patient medical, functional, and social needs.
  • Works with patients and families/caregivers to set goals and actions.
  • Collaborates with physicians and other members of the health care team.
  • Participates in rounds, case conferences, multidisciplinary groups, and guideline development.
  • Helps reduce length of stay and improve resource utilization.
  • Provides or facilitates education for physicians and ancillary departments/nursing units regarding case management and discharge planning.
