Case Manager
Overview
- Travel/Contract role in Mission Hills, California
- Start date: ASAP
- 13-week contract
- Day shift: 5x8 hours, 8:00–16:30
- 40 hours per week
- Ratio: 1:20
- Floating: No
- Weekend rotation required
- RTO limit: no more than 7 days
- First-time travelers accepted; clinical will screen first-time travelers
- Acute care facility
Required Qualifications
- Valid RN license in California
- Associate of Science in Nursing / Associate Degree in Nursing
- 2 years of case management experience
- Case management experience within the last year
- Experience in an acute care setting
- Acute hospital case management experience
- Discharge planning
