Care Coordinator

Overview

• Contract role in Chico, California
• 13-week assignment
• 40 hours per week
• 8-hour shifts, 8am - 4:30pm
• Weekdays and weekends

Required Qualifications

• Valid RN license in California
• Associate of Science in Nursing / Associate Degree in Nursing
• Case Management (1 year of experience)
• Case Management experience within the last year
• Experience in an Acute Care setting
• Acute Hospital Case Management Experience
• Care Manager experience
• BLS

Role Details

• Coordinates care and services for assigned patients across an episode of illness or treatment
• May be assigned to the Emergency Department
• Works with patients, families, physicians, the patient care team, and payors
• Evaluates utilization, admission/discharge criteria, observation/1-day stays, 30-day readmits, Medicare, diagnosis-related criteria, and outlier cases
• Advocates for patients and families throughout the episode of care
• Remains involved until discharge or transfer to acute care or another level of care
• Initiates clinical pathways based on admission diagnosis and serves as a resource for utilization management

Float / Staffing Notes

• Med/Surg and Critical Care RN positions are required to float as needed
• Agency staff must be W-2 employees of their agency
