RN Care Manager

Overview

Role: RN Care Manager
Location: Roanoke, Virginia
Employment type: Temp
Shift: Days
Hours per day: 8
Contract length: 13 weeks

Setting

Primarily works in the patient’s residence.
Travel within the entire hospice service area is necessary.
Travel to long-term care facilities, hospitals, and area agencies and businesses will be necessary.

Required Qualifications

Associate of Science in Nursing / Associate Degree in Nursing.
Valid RN license in Virginia.
BLS issued by American Heart Association.
Case Management experience within the last year.
2 years of Case Management experience.
Experience in an Acute Care, Geriatric, or Home Health setting.
Hospice experience.
Med Surg experience.
Driver's License.

Role Details

Provides patient care with symptom management, crisis intervention, and family education.
Requires written and verbal communication, problem solving, and decision-making skills.
Should be able to develop and maintain rapport with patients and families and work well with an interdisciplinary team.
