Job Opportunity: RN Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Summary

As a Home Health RN, you will:

• Assess patient conditions
• Implement care plans
• Provide ongoing education and support to patients and their families
• Perform admission processes and case management responsibilities
• Deliver patient care in accordance with community health practice standards and organizational policies
• Utilize thorough and timely electronic documentation and interdisciplinary communication
• Travel to assigned cases as needed
• Participate in weekend rotations approximately once every 6 weeks

Experience Requirements

• Minimum 1 year Certified Home Care experience
• Minimum 1 year Case Management experience

No prior travel experience is explicitly required.

Credential & Compliance Requirements

• Professional license verification (NURSYS and Board of Nursing required; MA Conditional Licenses not accepted)
• Submission of a recent professional photo (color, plain background, no filters or eyeglasses)
• Motor Vehicle Record (MVR) check within 45 days of start
• Criminal background checks including Massachusetts CORI, National Sex Offender, Exclusion checks, and Social Security Number trace
• Resume including past 7 years of work experience with city and state
• Two recent references (minimum one supervisor or above)
• Skills checklist required for submission
• Confirmation of assignment form upon acceptance and for extensions
• Education verification of highest level achieved
• Fit test for N95 respirator within 365 days of start, updated annually
• Respirator Medical Clearance attestation (if fit test required)
• Health requirements including:

  • TB test or negative QuantiFERON/T-spot within 3 months of start
  • Influenza vaccine during flu season (Sept 15 - April 30) with no exemptions
  • MMR vaccination or proof of immunity
  • Health statement signed by a licensed provider within 365 days
• Optional but strongly preferred: Hepatitis B and Tdap vaccinations
• Additional health screenings and documentation as required by Massachusetts Department of Public Health

Work Schedule

• Weekend rotations approximately once every 6 weeks
