Job Summary

As a Home Health RN, you will:

• Assess patient conditions
• Implement care plans
• Provide ongoing education and support to patients and their families
• Perform admission processes and case management responsibilities
• Deliver patient care in accordance with organizational philosophy, policies, and community health standards
• Utilize thorough and timely electronic documentation and interdisciplinary communication
• Travel to assigned cases as needed
• Participate in weekend rotations approximately once every 6 weeks

Required Experience

• Minimum 1 year Certified Home Care experience
• Minimum 1 year Case Management experience

• 13 weeks

Credential & Compliance Requirements

Required for Submission

• Professional License/Certificate Verification (Primary source verification; MA Conditional Licenses not accepted; both NURSYS and BON required)
• Resume including past 7 years of work experience with city & state
• Two recent professional references (minimum one supervisor or above)
• Skills Checklist

Required for Onboarding (must be completed prior to or at start)

• Facility Badge Photo (color photo, taken within past 6 months, no selfies or filters, plain background, no eyeglasses)
• Motor Vehicle Record (MVR) Check (within 45 days of start)
• Fit Test for N95 Respirator (within 365 days of start, updated annually; no religious exemptions)
• Nurse Aide Registry Check (within 30 days of start; required by MA DPH for all employees)
• MMR Vaccination or Proof of Immunity (measles, mumps, rubella)
• Influenza Vaccine (required during flu season Sept 15 - April 30; no exemptions)
• TB Test (skin test or blood test within 3 months of start; updated if >6 month break)
• Health Statement signed by MD/DO/NP/PA (within 365 days of start; updated if >6 month break)
• Criminal Background Check (within 45 days prior to start; includes international searches if applicable)
• Massachusetts CORI (criminal history check within 30 days prior to start)
• Respirator Medical Clearance Attestation (part of Fit Test record)
• Confirmation of Assignment/POI Form (for initial assignments and extensions)
• Education Verification (highest level of education verified through background check)
• Social Security Number Trace (addresses/aliases for past 7 years)
• National Sex Offender Check (within 30 days of start, updated annually)
• Exclusion Checks (OIG, GSA, MassHealth & NH Medicaid lists; within 30 days of start and annually)

Optional / Preferred

• Hepatitis B vaccination or proof of immunity or declination
• Tdap vaccination within last 10 years (preferred)
• Positive PPD with negative chest X-ray documentation (if applicable)
• TB Symptom Survey (if positive PPD)

Additional Notes

• No religious or medical exemptions allowed for fit testing or influenza vaccine
• Fit testing cannot be performed onsite; must be completed prior to start
• Weekend rotations occur approximately once every 6 weeks
