Job Opportunity: RN Home Health (LTA)

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Summary

As a Home Health RN, you will:

Assess patient conditions
Implement care plans
Provide ongoing education and support to patients and their families
Perform admission processes and case management responsibilities
Deliver patient care following organizational philosophy, policies, and community health practice standards
Utilize thorough and timely electronic documentation and interdisciplinary communications
Participate in weekend rotations approximately once every 6 weeks
Travel to assigned cases as needed

Required Qualifications

Minimum 1 year of Certified Home Care experience
Minimum 1 year of Case Management experience
Valid RN license for Massachusetts

Additional Information

Start date contingent upon credentialing clearance
Work locations include various neighborhoods in the Boston area (e.g., Back Bay, Beacon Hill, Fenway)
